This is an opportunity to step into a senior clinical support role within a structured care model where your expertise will directly influence safe, compliant and person-centred care delivery.

Please note: This role is office-based in Bowen Hills and requires attendance in the office five days per week.

The Role

The Pod Clinician plays a key role in supporting safe, effective and person-centred care delivery for clients within a structured phone-based care environment. Operating as the Pod Clinician, this role provides senior clinical support across Home Care Package and Support at Home program services.
You will apply clinical expertise and judgement to support care planning, clinical reviews, complex risk assessment, escalation, incident management and overall client outcomes, while also providing clinical guidance to Care Partners and Team Leaders.

Working closely with Care Partners, Team Leaders, clients and their representatives, and in partnership with Clinical Governance, you will support clinically informed decision-making and contribute to safe, compliant and high-quality care delivery aligned with aged care legislation, professional standards and internal processes.

Key Responsibilities Clinical Care and Support
  • Provide clinical support across Support at Home program services within a phone-based care model
  • Support care planning, clinical reviews, monitoring and reassessment of client needs
  • Identify changes in presentation, clinical risks or deterioration and respond appropriately
  • Escalate clinical concerns in line with procedures, professional obligations and legislation
  • Maintain accurate and timely clinical documentation including assessments, reviews and escalations
Clinical Advice and Decision Support
  • Provide clinical guidance to Care Partners, Team Leaders, clients and their representatives
  • Support clinically informed decision-making around care planning and service suitability
  • Review complex client matters and contribute to person-centred clinical recommendations
  • Promote consistent clinical practice aligned with professional standards and aged care obligations
  • Escalate high-risk or complex matters to relevant internal stakeholders
Care Quality and Compliance
  • Support care delivery in accordance with the Aged Care Act, Strengthened Aged Care Quality Standards and Support at Home obligations
  • Contribute to identifying and managing clinical and care-related risks
  • Ensure high quality documentation and adherence to internal escalation pathways
  • Support implementation of clinical practice improvements and workflows
The successful applicant will have
  • Strong clinical assessment, critical thinking and professional judgement
  • Ability to provide clinical supervision and guidance in a structured office-based and phone-based care environment
  • Sound understanding of clinical risk, incident management and safe care practices
  • High-level written and verbal communication skills
  • Strong professional writing and documentation capability
  • Ability to work independently while collaborating effectively across teams
  • Strong organisational skills and ability to manage competing priorities
  • Proficiency in Microsoft 365 and CRM/documentation systems
  • Commitment to person-centred care and professional accountability

Mandatory Qualifications and Experience
  • Tertiary qualification in healthcare
  • Current unrestricted AHPRA registration with the relevant governing Board
  • Minimum 5 years' clinical experience, preferably in aged care, home care or community health
  • Demonstrated experience in clinical risk, incident management, audits or compliance-related practice
