Case Manager – Outreach

• ASAP Start | Contract until end of June
• Full-time (5 days per week)
• Northern Suburbs, Sydney

We are currently seeking an experienced Mental Health Outreach Worker to join a well-established Psycho-social Support Service across Northern Sydney. This is a fantastic opportunity to make a meaningful impact by supporting individuals in the community with mild to moderate mental health presentations.

About the role:

• Manage a caseload of up to 20 clients
• Provide community-based outreach support
• Deliver strengths-based, recovery-oriented case management
• Support clients to achieve goals and improve independence
• Work collaboratively with internal teams and external stakeholders

About you:

• Strong case management experience within mental health or community services
• Experience supporting clients with mild to moderate mental health needs
• Confident working autonomously in a community-based role
• Excellent communication and engagement skills
• Current driver's licence (pool cars provided)
